Evaluation of Guelph and Wellington County’s Integrated Youth Service Network Hubs
The Integrated Youth Services Network (IYSN) is a collaborative project that provides mental health and other supports to help youth build bright futures. In Guelph and Wellington, the IYSN has seven sites or hubs to improve access for individuals in rural areas. All hubs are connected through an e-Health system to provide seamless service delivery across the region.
HRI has partnered with the IYSN to develop and implement a framework to evaluate the IYSN model of care. The evaluation will provide evidence about the early experiences of youth using IYSN services. It will also inform continuous improvement activities and assess the IYSN model for replication in other sectors or areas of healthcare. The first phase of this project focuses on planning and the development of evaluation tools.
